Approximately 6 or 7 years ago, in Chicago, there was an occurrence where an airplane took off without receiving clearance from ATC, which I still remember today. In this case, an Eagle Airways flight was holding short of the runway and waiting for clearance to take off. Without any communication with the tower or ATC clearance, the pilot suddenly took off, crossing the departure end of another active runway, causing an A380 to take emergency evasive action to avoid a collision. The incident forced the A380 to enter an unexpected go-around, resulting in significant additional fuel burn, delays, and potential additional safety risks.
